WHAT IS HOME HEALTH CARE?

Home health care brings skilled medical services directly to you wherever you call home. Our team works closely with you and your loved ones to support recovery from illness, surgery, or injury, or to just assist with age- related challenges.

Your care is guided by a personalized plan developed in coordination with our clinical team. Depending on your needs, services may include:

WHO IS ELIGIBLE FOR HOME HEALTH CARE

You may qualify for home health care through Medicare if the following apply:

  • Your doctor has recommended home health services as part of your care plan

  • You need skilled nursing care or therapy (such as physical, occupational, or speech therapy) on an intermittent basis

  • Your health condition makes it difficult to leave home without assistance, meaning you meet Medicare’s definition of being “homebound”

WHEN WILL MY HOME HEALTH VISITS START?

A nurse or therapist will call to schedule your first visit.
If you’re being discharged from a hospital or nursing facility, your initial home health visit is going to take place within 24- 48 hours to ensure a smooth and timely transition to care at home.

IS HOSPITALIZATION A REQUIREMENT TO RECEIVE HOME HEALTH CARE?

No. Hospitalization is not a requirement to receive home healthcare. Many patients are referred by their doctor 

IS HOSPITALIZATION A REQUIREMENT TO RECEIVE HOME HEALTH CARE?

Homebound means your condition is such that:

  • You’re normally unable to safely leave your home without help from others and the aid of assistive devices (such as crutches, canes, walkers or wheelchairs).

  • Leaving home would require considerable and taxing effort. You can generally leave home as often as you need for medical treatment that cannot be provided in the home and still be considered homebound. You’re also allowed brief absences from the home for some non-medical reasons, such as an occasional trip to the barber or beauty shop, to attend church, or for unique family events (like a graduation or wedding). Such trips must be infrequent and require a considerable and taxing effort. 

HOW LONG WILL I RECEIVE HEALTHCARE? 

Your doctor will determine the number of visits you receive, how often the visits should occur and ho long they should last, based on your needs and health status.
