The specific Home Health Care services provided are intermittent visits

by the following:

  • Skilled Nurses

  • Physical Therapists

  • Speech Language Pathologists

  • Certified Nurse Assistants

  • Social Workers

  • Occupational Therapists

 

 

MEDICARE COVERAGE GUIDELINES:


Medicare requires that you meet the following criteria:

You must be homebound. This means that due to your illness or injury there is an inability to leave the home and doing so would require a considerable and taxing effort.

You can still be considered homebound if you:

​

  • Leave your home infrequently or for a short duration.

  • Receive medical treatment, therapeutic, or psychosocial activities in an accredited adult day care program.

  • Leave your home only to attend religious services.

  • Leave your home only to go to the beauty salon or barbershop.

  • You must have had a recent illness or injury, which requires skilled nursing, physical therapy, and/or speech language pathology on an intermittent basis, or have a continuing need for occupational therapy.

  • You must be under the care of a doctor who has ordered the treatment and services that we are providing. In order for Medicare to cover the services provided, they must be reasonable and necessary.

  • A person with a psychiatric illness may be considered homebound even if there are no physical limitations. The nurse will explain specific criteria that apply to psychiatric diagnoses.
